We will take the required time to properly diagnose your immediate dental concerns, review your past medical and dental history, and create a treatment plan that ensures your optimal dental health. With your cooperation, we will complete a thorough, comprehensive dental evaluation.

Preparing for your first appointment

Your initial appointment usually consists of a comprehensive examination, cleaning, any necessary X-rays, and a review of your treatment options.

Please assist us by providing the following information at the time of your consultation:

  • Any x-rays taken by a previous dentist, if applicable.
  • A list of medications you are presently taking.
  • If you have dental insurance, bring the insurance card(s). This will save time and allow us to help you process any claims.
  • New patient forms that need to be completed will be sent to you electronically. Please make sure they are completed before your visit.

IMPORTANT: All patients under 18 must be accompanied by a parent or guardian.

Please alert the office if you have a medical condition that may be of concern before treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your previous dentist has taken x-rays, you may request that they forward them to our office at: [email protected]

If additional x-rays are necessary, they will be taken at our office.
